The first step of adding survey questions with the Question Wizard is to select questions to add to your survey.

On the Welcome screen of the Question Wizard, choose a survey from the drop-down and click ‘Next’ at the bottom of the screen.

Browse Questions

The ‘Add Questions’ screen contains the library of questions available to add to your survey. Browse the entire list of questions or use the fields above the list to search by keyword or category, or to filter by answer type. By default, ten questions display initially. Use the ‘Load More’ button at the bottom of the screen to view more questions.

A colored circle next to any question indicates it is a gate question. The questions triggered by these gate questions are represented by an arrow of the same color.

To see the response options for any question, click on the question text. For each question you can view the category, answer type, and total number of responses nationally for the past 90 days.

Add Questions

To add a question to your survey, click the checkbox for that question under the ‘Select’ column. You may add as many questions as you like. Once you’ve selected at least one question, all questions you’ve chosen will display at the bottom of the screen under the ‘Selected’ heading. To deselect a question, click the checkbox again.

When you are finished selecting questions, click ‘Next’ at the bottom of the screen.

Low Score Comment Prompt

Certain questions have the option of adding a low score comment prompt. When added, the patient will be prompted to leave a comment when they select a low score (a 1 or 2) on a 5-point Likert scale question. On the ‘Low Score Comment Options’ screen, click the checkbox for any question to add this prompt. Click ‘Next’ at the bottom of the screen to proceed to selecting your filter options.
