After you choose questions to add to your survey in the Question Bank, you can specify any filters you’d like to apply. Entity filters deliver questions for specified divisions and/or providers in your organization.

Select Entities by Name

In the ‘Division(s)’ and ‘Provider(s)’ boxes, click the ‘Select’ button to browse or search for specific divisions or providers. Questions will be delivered on surveys for these entities (when any additional filter criteria are met).

Selecting Divisions

Use the checkboxes to select one or more divisions. You may search for a division using the fields above the list.

Filters currently assigned to a division will display in the Active Filters column.

The 30 Day Composite Score column shows the division’s composite mean for the past 30 days.

Selecting Providers

Use the checkboxes to select one or more providers. You may search for a provider using the fields above the list or use the drop-down to filter by specialty.

Filters currently assigned to a provider will display in the Active Filters column.

The Divisions column displays the divisions the provider is associated with. When you select a provider, a drop-down will display in this column if you wish to send questions to only specific practice locations for this provider.

The 30 Day Composite Score column shows the provider’s composite mean for the past 30 days.

When you are finished selecting your entity filters, click ‘Next’ at the bottom of the screen. You will navigate to the next applicable section of the Question Wizard based on your filter type selections.
